ABOUT US

Finger Lakes Coordination & Care was founded in 2019 to provide Service Coordination to participants in the New York State Department of Health’s Traumatic Brain Injury (TBI) and Nursing Home Transition & Diversion (NHTD) Waiver Programs.

Together, both of our Service Coordinators have over 30 years of experience working with people with disabilities and over 10 years of combined experience working with the TBI and NHTD Waivers.

We have a passion for our participants- this agency exists to better the lives of those people that require a higher level of care. We do this by listening to each person and their family and collaborating with their care team to bring them the services they need to meet their personal goals while maintaining their health and safety.
