Our Services

Finger Lakes Coordination & Care (FLXCC) provides Conflict-Free Service Coordination to work with you and your family to assess what services you need in order to stay in your home. Once that assessment is complete, we will work diligently to refer you to providers of those supports identified to keep you healthy and safe in your own home.

What Does “Conflict-Free” Mean?

FLXCC is the first local agency to offer conflict-free Service Coordination. This allows us to assess your needs and coordinate services for you that are separate from our own agency. This eliminates any biases and ultimately promotes your choice and independence.

We Work For You

We do not believe in giving ourselves a flat salary and getting paid regardless of the work that we do. At FLXCC we don’t get paid unless you’re living in your home. This means we will work hard to get you discharged from the hospital or nursing facility when it’s time and make sure you have all the supports you need to continue to remain in your home for as long as possible.
